The Comptroller and Auditor General (CAG) of India's latest report has uncovered critical deficiencies in the management of health services in Indian Railways. The audit found acute shortages of both medical and paramedical staff, specifically in hospitals within the Zonal Railways, reflecting a dire need for staff recruitment.

The report also highlighted substantial gaps in the availability of medical equipment, failing to meet even the minimum requirements set by Indian Public Health Standards (IPHS) and the Clinical Establishment Act of 2010.

Despite Indian Railways' extensive healthcare network serving nearly one crore beneficiaries, the audit exposed shocking inefficiencies. The audit period spanning from 2017 to 2022 revealed a significant increase in healthcare expenditure but noted a steep decline in 2021-22. Alarmingly, numerous patients left hospitals against medical advice, raising serious questions about the quality and adequacy of care.

Specific findings included the non-implementation of Ayushman Bharat Yojana in many Railway Hospitals and a below-standard Bed Occupancy Ratio (BOR). Additionally, a vast majority of medical referrals to non-railway hospitals lacked necessary audits, compromising patient safety.

The audit recommends stronger monitoring mechanisms, timely procurement, and implementation of the Hospital Management Information System (HMIS) to address these gaps. The CAG emphasizes the urgent need for comprehensive reviews and periodic assessments to improve health services for railway employees and their families.
