The report, signed by five rail officials, pinpointed miscommunication between pointsmen Amar Kumar and Mohd Suleman. Suleman's wrong signal to the loco driver caused the fatal error, resulting in Kumar's death. A video of the incident quickly spread on social media.
In defense, Suleman argued the loco driver was at fault as he reversed the engine without receiving a hand signal. The tragic accident occurred at 8:29 am, when Kumar got trapped between the buffers. An additional senior-level inquiry is underway.
