High-Level Probe Blames MMRCL and Contractor for Fatal Andheri Incident
A probe committee appointed by the Brihanmumbai Municipal Corporation (BMC) has found Mumbai Metro Rail Corporation Ltd and its contractor culpable for the death of a woman who fell into a storm water drain during heavy rains in Andheri. The report cited the MMRCL's responsibility to address site deficiencies.

A Brihanmumbai Municipal Corporation (BMC)-appointed high-level probe committee has blamed Mumbai Metro Rail Corporation Ltd (MMRCL) and its contractor for the tragic death of a woman who fell into a storm water drain during last week's downpour.
In its release on Monday, the BMC cited the committee's findings, noting that the responsibility for rectifying faults at the accident site, located in Andheri in Mumbai's western suburbs, lies with MMRCL and its contractor, L&T. The location had been under their control since 2015.
The incident occurred on September 25, when 45-year-old Vimal Anil Gaikwad fell into the drain amid heavy rains. A three-member committee was subsequently appointed to investigate the incident.
The BMC had earlier informed L&T of deficiencies at the site and had sent letters to MMRCL in August 2024, urging corrective measures. Ashwini Bhide, managing director of MMRCL, acknowledged the need for a thorough review.
The probe also criticized BMC officials for not taking adequate steps to address the issue, emphasizing the need for greater vigilance, especially given the heavy rainfall and main road location.
The panel was led by Deputy Municipal Commissioner Devidas Kshirsagar, along with Mumbai Fire Brigade chief Ravindra Ambulgekar and Chief Engineer (Vigilance) Avinash Tambewagh.
